I am always honest in responding to what I'm currently being paid. I'd rather set a good tone with an employer.

When asked about salary, I usually respond that I'm being paid X, and were I to be made an offer I would expect that it reflect any changes in responsibilities and cost of living (if moving to a new area). That sort of puts it in their court, and forces them to make the first move. Then you could negotiate up from there by quoting what you know about market rates and such.
