So you are getting a horizontal rule above the title and another below the
footnote, and you don't want these two horizontal rules, right?

They may come from the ODS style you are using. Please show your code.

Presumably your output is going to be processed with MS Word. You may find
it easier to implement the "1 of 3" etc. in Word than in SAS.
