Yes, you are correct on both counts. Sorry for the trouble you had determining what the output should look like. I am trying to code most significant source of HIV exposure. The hierarchy from top to bottom is:

MSM/IDU MSM IDU Blood Endemic Heterosexual Other/Unknown

So, for example, if a patient has exposure via:

1) men having sex with men and intravenous drug (MSM/IDU) and 2) Immigration from an HIV endemic region (Endemic),

they will be assigned the code MSM/IDU.
