I'm a little curious why one would fill in a form like this from a dataset
since it appears to be designed to be filled in by the user.
Getting on to your actual question, I have a few ideas.
First, you should be able to use a put statement and rtf and, if you hade
code on the mainframe, you should be able to use that on the pc provided
you change the outpfile names to confrom to windows. To test a put and rtf,
ods rtf body = 'c:\puttest.rtf';
put 'This text was sent to RTF via ODS and a Put Statement';
ods rtf close;
Second: Do you, by chance, have SAS/FSP licensed? You could use Proc FSEDIT
in batch mode (it will print each obs using the layout from your screen
catalog) or you could use Proc FSLetter. Either of these would do what you
Third: There is a product called Omniform which can be used to scan an
existing form and set up a template with editable fields. The HR department
in my company has our medical forms set up with it so we can call up the
form on our intranet and fill it out and print it. It's been some years
since I played with it by the product is supposedly ODBC compliant so you
"should" be able to fill out a form from a SAS data set. ODBC and I have
never really become friends so I can do little more that suggest the

I've been trying to find samples of code to solve this issue but I can't.
I have been asked to recreate a form and populate it with the
appropriate data. That's fine and I used to do something similar on the
mainframe using PUT @ statements and column counting. However, what I
would like this time is to do something similar but output to either PDF
or RTF. My data is formatted such that each row would be self-contained
withing the page, next row - next page.
Although this is not the form I need to re-create, the format is similar
to page 1 of this form.
Do anyone have any sample code or could direct me where to find such code?
