 Dear December Graduation Candidates,

We did not realize that an upgrade to the forms last night would create new Web addresses for them.  The new link for the Application for Graduation is

The new link for the Doctoral Commencement Information form is
http://gradschoolforms.webapps.uga.edu/form_types/2

If you submitted your form successfully (got a confirmation) before 5:00 yesterday, we received it.
The link for all the other forms that must be printed and sent to us is the same:
