'Questionnaire' and 'survey' are both defined in:
A Dictionary of Statistical Terms
M G Kendall & W R Buckland
(4th Ed 1982) Longman: New York & Harlow UK.
The definitions are along the lines suggested in earlier emails, except
K&B point out that 'survey' is usually assumed to mean 'sample survey'.
cf also, 'census'.
