Posted to several lists; we apologize for duplication.
 
A service committee at the University of Oregon Library is
looking for libraries that have foolproof reference referral
processes in place.  Okay, so yours isn't perfect, but we want to
hear how your library deals with the challenges of referring
library users to other libraries or library departments when your
department can't meet their needs.
 
1.   What process do you use?
2.   Do you use standard referral forms?
3.   How do you train staff to do accurate referrals?
4.   Are staff issued written guidelines to assist them in
     referrals?
5.   Any comments on successes or failures -- we're happy to hear
     your horror stories if you think we might learn from them.

results for the list if there is interest.  If replying by
