Nevermind, I see there is also a free toolbar available.  But they are a
little sneaky because Acrobat tries to get you to install the Trial version
of the $500 commercial product instead of pointing you to the free toolbar.

Still would be nice if there was a free way to convert the PDFs to GeoTIFFs.
I hope the USGS saved taxpayers a ton of money by going with the proprietary
format.
