Try the Epidemiology Program Office, Centers for Disease Control and
Prevention, Mailstop C08, Atlanta, GA 30333. Phone (404) 639-1326, FAX (404)
639-1546.
 
They probably have such a map in digital format generated on their EpiMap
software.
 
Anticipating the next question, the EpiMap software is available by ftp:
 
ftp:src.doc.ic.ac.uk@computing/systems/ibmpc/simtel20/mapping
 
(at least this was correct as of last March)
