Your surest bet for a North American raised relief is Nystrom (in Chicago--they
have a 1-800 number). The other two major US manufacurers, Kistler and Hubbard
Scientific, do not, I believe have full North America pieces.
 
There is at least one German source for raised relief maps (Perthes?), and a
limited selection from IGN in France.
 
Finally, there were some interesting pieces I saw in Frankfurt in September
1991, produced by PPWK in Poland and the old Soyuzcarta in the USSR. No idea
what's become of them.
