As of about 5 years ago they made them by taking linking a hand-held pointer to an automatic drilling/milling machine - just like the old-fashioned (Thomas Jefferson era) signature duplicators. They would then take a topo map as a base and then hand-trace each major contour, producing a plaster-of-paris master.
