Staff artists and cartographic technicians at the National Geographic
Society created the shaded relief by hand when I worked there in the
'80s.  I suspect its done the same nowdays.

I have no idea how National Geogaphical make their shaded relief map,
i.e,
what software they use? what database they use? Do they draw it by hand?
