Hello all,
I've found the full citation and list of libraries for the question that I
asked yesterday (see below).  Don't know how I missed it in OCLC!  Thanks to
Gwen Curtis and Rick Seitz so far.
 
The answer is the OCLC number is 00128317.  Nine libraries report holding
it.
