It is Modulus calusa Petuch, 1988.

I've listed some S.E. U.S. (Florida) fossil Modulus species, if you'd
like them. My information is all from Petuch.

Modulus woodringi Mansfield, 1930 (APAC - Sarasota, FL)
Modulus basileus (Guppy, 1873) (Palm Beach Co., FL)
Modulus caloosahatcheensis (Petuch, 1994) (Palm Beach Co., FL)
Modulus bermontianus (Petuch, 1987) (Capeletti Pit, Dade Co. FL)
