I was hoping and looking for someone to answer your request, as I'm sure
there is a standard.  I would check with my university-librarian buddy but
he is out looking for birds this weekend.

Because our newsletters have a very forgiving audience, I just list the web
page by: author, date of the page (if it has one, if not then the date you
read it), title of the page (if it has one), then the statement "Internet
document at:" followed by the web page URL.  The kicker is when something
happens like, the page is now gone but you copied the relevant material onto
your hard drive.  In that case (which just happened to me) I include a
statement relating that fact.  I believe giving credit and providing a
verifiable source (if possible) are more important than format.  We should
probably save to disk any web page source .
