In answer to your question on where to get the US Board on Geographic
Names Gazetteers.  They are available as hardcopy publications from the
USGS at:

A commercial digital version is available.  GDE Systems bought a copy (Feb
95) of the database used to generate these gazetteers and produced a
digital gazetteer CD which contains all 4 million current names.  (Some
"current" hardcopy gazetteers are over 40 years old.) This CD is for an
IBM-compatible PC running MS Windows 3.1/3.11.  Please contact us by email
